Requirements

- Patients must be 0–19 years old
- Maryland Healthy Smiles (Medicaid) patients must confirm eligibility and maintain benefits
- After two no-shows or same-day cancellations, Maryland Healthy Smiles patients will be asked to find another dentist
- Assignment of benefits accepted only if insurance information is provided prior to appointment
- Self-pay patients must pay in full at time of service
