- Enrollment in HealthPartners PMAP, MSC+, SNBC, or MSHO, or being a pregnant woman or child in MinnesotaCare
- Must be unable to drive or lack other transportation
- Only medical-related trips to approved providers (doctor, dentist, mental health, urgent care, specialty clinic, hospital)
- Advance scheduling required based on appointment type and location
