Consultations and treatment by specialist doctors for conditions such as asthma, diabetes, or heart problems, available by referral from your Medical Home.

Confidential individual counseling for depression, anxiety, panic, or other emotional problems, available through your Medical Home or San Francisco Community Behavioral Health Services.

Confidential services for alcohol or other drug problems provided through San Francisco Community Behavioral Health Services.

Services including birth control, pregnancy testing, and sexually transmitted disease testing, available at most Medical Homes.

Prescribed equipment such as wheelchairs or crutches, arranged through your Medical Home.

Medical or laboratory tests performed at your Medical Home or at hospitals in your Medical Home Network.
