- Youth Support Outreach Program limited to ages 12–17
- Project CHILD limited to pregnant people using substances or in active recovery
- Must reside in Hennepin County
- Information about the family (names and addresses of child and parents)
- Specific description of suspected abuse or neglect (what happened, when it occurred, identity of abuser)
- Child’s school, other witnesses, child’s location, names of other family and household members
- Online reporting form or faxed printable form for child protection reports
- Online eligibility form for Adult Representation Services
