Requirements
- new patients accepted
- Must reside in the seven-county Twin Cities metro area
- PCA services require enrollment in a Minnesota Health Care Program (Medical Assistance, waiver, Alternative Care, or participating health plan)
- Individuals under 18 or those needing help directing their own care require a responsible party
- PCA cannot be the recipient’s parent, stepparent, legal guardian, spouse, responsible party, or another PCA recipient
- Sliding fee scale applies; reduced or no-fee services based on income
- Completed service referral form
- PCA Assessment and Service Plan (DHS-3244)
