Requirements

  • new patients accepted
  • Must reside in the seven-county Twin Cities metro area
  • PCA services require enrollment in a Minnesota Health Care Program (Medical Assistance, waiver, Alternative Care, or participating health plan)
  • Individuals under 18 or those needing help directing their own care require a responsible party
  • PCA cannot be the recipient’s parent, stepparent, legal guardian, spouse, responsible party, or another PCA recipient
  • Sliding fee scale applies; reduced or no-fee services based on income
  • Completed service referral form

  • Background check for Personal Care Assistants
