245D: Home Community Based Service

Waiver services to help people maintain community independence. MNCCS is a 245D HCBS provider and has a contract with Ramsey County to provide waiver case management for over 65 people. Includes operation of a child foster care respite home in Bloomington providing up to three days of overnight respite care for children with autism, serving both the East African immigrant community and the general public.

Services for people experiencing homelessness. MNCCS provides Housing Supports through contracts for 150 scattered site units in four counties and offers Housing Stabilization Services through insurance. Expertise includes outreach, transition support, and tenancy sustainment.

Includes ARMHS (Adult Rehabilitative Mental Health Services) in home and community settings across the metro area, Faribault, Owatonna, and Rochester; trauma-informed, culturally responsive psychotherapy; medication management; practitioner supervision; and assistance with disability assessments.

Out-of-Home Respite Under Children Foster Care License

Newton out-of-home Respite Program in Bloomington for up to four youths (up to age 18) at a time. Provides a therapeutic, supervised environment with recreational activities, well-balanced meals made from mostly non-processed foods, and social engagement, offering families and care providers a safe respite option.
