- Telehealth excludes all physical exams (sport, pelvic, genital, rectal, breast), STD/STI screenings, UTI evaluation in males, and any visit requiring lab testing
- Telehealth excludes emergent symptoms (chest pain, shortness of breath, anaphylaxis, choking, altered consciousness, moderate/severe abdominal pain)
- Employer Authorization for Examination or Treatment (occupational health)
- DOT Physical Exam form (must be completed on day of exam)
- Company-specific occupational health forms (if applicable)
- Federal/state forms (available online)
