Requirements

- Must have funding through Home and Community-Based Waivers or private pay
- Individuals must be seniors or have a qualifying disability
- Services limited to Hennepin, Anoka, Ramsey, and Scott counties
- Consultation services available only to those transitioning to CFSS
- Approval for Home and Community-Based Waivers (DD, CADI, BI, EW, AC, CAC)
- Enrollment in Consumer Directed Community Supports (CDCS) if applicable
- Private pay arrangements (if not waiver funded)
- Completed CFSS support plan for Consultation Services
