Referrals to mental health counseling, substance use treatment programs, and housing services.

Transport provided by Street Crisis Response Team, SF Homeless Outreach Team, or ambulance services.

Ongoing monitoring of vital signs to ensure participant health and safety.

Screening for signs of medical emergencies and triage to appropriate care.

Continuous observation for overdose symptoms with intervention as needed.

Assessments for substance use history, physical and mental health conditions.
