Requirements
- new patients accepted
- Must be a health care professional regulated or licensed in Minnesota
- Must have a potentially impairing condition (e.g., substance use or mental health disorder)
- Not open to individuals who are not licensed health professionals in Minnesota
- Completed Third Party Referral Form (for board or third-party referrals)
- Self-referral intake documentation (call or email case manager)
- Quarterly compliance reports
- Monthly medication logs
- Signed authorizations and consent forms
