Requirements

• new patients accepted
• Must have a serious mental illness diagnosis
• Must be receiving inpatient or residential mental health or substance use disorder treatment for 90 days or less (partial hospitalization programs are also eligible)
• People receiving outpatient treatment do not qualify
• Household income must be at or below 80% of the HUD area median income
• Must require support to maintain community-based housing
• Verification of serious mental illness diagnosis
• Verification of inpatient or residential mental health or substance use disorder treatment (90 days or less)
• Household income documentation (bank statements or proof of SSI/SSDI/MSA, etc.)
• Documentation of housing and utility–related costs
