- Must be a legal Maryland resident and provide proof of residence
- Must complete state registration (email verification, photo ID, photo upload). Processing takes 10–45 days
- Insurance does not cover the physician consultation or state application fee
- Physician visit fee is required
- Government-issued photo ID with Maryland address
- Photo of patient against a white background
- Valid email address
- Supporting medical documentation (records or medication history)
- Completed patient intake form
