Requirements

  • new patients accepted
  • Age between 50 and 80 years
  • Current smoker or former smoker who quit within the past 15 years
  • Smoking history of at least 20 pack-years
  • No signs or symptoms of lung cancer
  • Insurance plans may have their own age or coverage limits for CT lung cancer screening
  • Photo I.D.
  • Health insurance card
  • Insurance authorization, if required
  • Doctor's referral, if required
  • Recent test results related to your condition
