Requirements
- new patients accepted
- Age between 50 and 80 years
- Current smoker or former smoker who quit within the past 15 years
- Smoking history of at least 20 pack-years
- No signs or symptoms of lung cancer
- Insurance plans may have their own age or coverage limits for CT lung cancer screening
- Photo I.D.
- Health insurance card
- Insurance authorization, if required
- Doctor's referral, if required
- Recent test results related to your condition
