Requirements

- Must have been born with a complex birth defect
- Conditions served include congenital diaphragmatic hernia, anorectal malformations, Hirschsprung's disease, esophageal atresia, congenital lung masses, sacrococcygeal teratoma, biliary anomalies, and anterior abdominal wall defects
- Patients must be infants through teenage years
- Photo I.D.
